VARICOCELE INCREASES THE RISK OF INFERTILITY IN MEN

Stating that varicocele is a clinical name given to the varicose veins of the testicles in the legs, Dr. said, “Varicocele, one of the most common causes of male infertility, is a correctable condition. While it is seen in the general population at an adult age of about 15-20 percent, varicocele is detected in 25-40 percent of men investigated for infertility,” he said.

Speaking about the symptoms of varicocele, Dr. said, “Swelling, swelling, pain in the testicle may be seen on the testicle. The dilation in the veins may become noticeable enough to be seen from the outside over time and may take on a similar appearance to varicose veins on the legs. Symptoms of varicocele include swelling in the testicles, as well as sweating and a feeling of warmth. Decoction of the testicles is a common symptom of varicocele. In rare cases, testicular shrinkage may occur,” he said.

A varicocele can be noticed by careful people who examine it due to irregularities, swelling or pain on the testicle, says Dr. said, “Varicocele is usually diagnosed during the examination in those who go to the doctor with infertility complaints. If pain occurs after situations requiring exertion, such as standing for a long time, sports or sexual activity, this may also be signs of varicocele,”he said.

Diagnosis and evaluation process

Emphasizing that the most important method for diagnosing varicocele is the examination to be performed by an experienced urologist, Dr. said, “The examination is usually performed standing up. Physical examination is the most valuable method. As a result of the examination, the diagnosed varicocele is divided into three degrees.

Grade 1- It is the lightest grade. During an outpatient examination, veins can only be felt with maneuvers such as coughing or straining.

Grade 2- It is moderate. During the outpatient examination, there are enlarged veins that can be felt manually without the need to increase the intra-abdominal pressure.

Grade 3- It is the most severe grade. Varicose veins are visible standing up. They can be felt by hand, especially in cases that increase intra-abdominal pressure. With this examination, factors such as the diameter of the affected vessels, whether the blood has returned, and the testicular volume can be determined. Subclinical varicocele is the name given to the condition of varicocele, which cannot be detected on physical examination but is diagnosed by radiological methods. The effect of treatment of subclinical varicocele on sperm parameters and pregnancy rates has not been conclusively proven,” he explained.

“Varicocele treatment methods”

Underlining in which cases varicocele should be treated, Dr. said: “Having a varicocele that can be manually examined, the couple has a known infertility complaint, the female partner’s fertility is normal or a cause of infertility that can potentially be corrected, the male partner has one or more abnormal sperm parameters (impaired sperm count, motility or morphology) or sperm function tests. The most accepted treatment method in the world for patients who have decided to correct varicocele disease by surgery is microsurgical (microscopic) varicocele surgery. Vascular structures are seen better with magnifications up to 15 times using an operating microscope, and venous structures that need to be eliminated due to impaired blood flow are cleaned. The arteries, lymph vessels, and sperm duct that bring blood to the testicle are protected. With this method, the complication rates such as varicocele recurrence, artery injury and hydrocele formation after surgery (fluid collection in the testicle) are the lowest, and the improvement in sperm parameters after surgery and pregnancy rates are higher than other methods. After varicocelectomy surgery, daily tasks can be performed at home after 1 day of rest. sexual activation is allowed after 1-2 weeks, heavy work life and sports activities are allowed after 2-4 weeks.”
